Service Care Solutions is in search of a dedicated and experienced Community Support Coordinator to become a vital member of our Community team.

As a Community Support Coordinator, your primary responsibility will be to facilitate care and assistance for individuals (along with their caregivers and families) who reach out or are directed to adult social services from the community.

You will serve as the main community-oriented access point for inquiries related to adult social services and safeguarding issues.

Your role will encompass a variety of social service functions, including:

  • Conducting initial screenings and assessments
  • Developing support plans
  • Coordinating cases to ensure high-quality service delivery

Additionally, you will assist individuals in recognizing their strengths and available resources, aiming to delay or prevent the necessity for new or additional social service interventions, thereby empowering them to maintain their preferred lifestyle.

It is essential to demonstrate exemplary practices in engaging with adults through a strengths-based and person-centered approach, linking individuals with informal, community-based resources whenever feasible.

Your commitment to collaborating with residents, caregivers, community organizations, and other key professionals is crucial in promoting independence for those you serve.

Prior experience working with vulnerable populations and a solid understanding of adult safeguarding principles are required.
