Position Overview

We are seeking a dedicated and proactive individual to become a vital member of the Discharge team at Buckinghamshire Healthcare Trust. This team plays a crucial role in facilitating the discharge process for patients with diverse and complex needs.

In this role, you will collaborate closely with experienced discharge coordinators to assist in the planning, coordination, and safe transfer of care for patients with intricate discharge requirements, from their admission through to their effective discharge.

Key Responsibilities

Facilitate the delivery of high-quality discharge services and patient flow for individuals with complex discharge planning needs, prioritizing the requirements of patients and their caregivers, and acting as their advocate when necessary. Collaborate with ward personnel to identify patients requiring complex and multi-faceted discharge planning interventions, ensuring timely assessments and referrals are made to meet the patients' needs and prevent delays. Engage and communicate effectively with patients, their families, caregivers, and staff from community services, social services, and voluntary organizations to formulate an optimal discharge plan from admission to discharge.
