Nurse - Acute Care Specialist

We are seeking a highly skilled and dedicated Registered Nurse to join our team in an Acute hospital environment. As a key member of our healthcare team, you will be responsible for providing high-quality patient care, working collaboratively with other healthcare professionals to ensure the best possible outcomes.
Key Responsibilities- Assess and monitor patient conditions, identifying and responding to changes in their health status
- Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans
- Work effectively with multidisciplinary teams to coordinate patient care, ensuring seamless transitions and optimal outcomes
- Communicate effectively with patients, families, and healthcare teams, providing clear and concise information and education
- Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices

Requirements- A minimum of 6 months paid experience in an Acute hospital environment within the last 3 years
