About the Role

Lakeside Healthcare Group is seeking a highly skilled and experienced Practice Nurse to join our team. As a Practice Nurse, you will play a vital role in delivering high-quality primary care services to our patients.

Key Responsibilities
  • Provide comprehensive primary care services, including health education, disease prevention, and management of chronic conditions.
  • Work collaboratively with the multidisciplinary team to promote integrated pathways of care and improve patient outcomes.
  • Develop and implement health promotion activities to enhance patient engagement and empowerment.
  • Conduct initial assessments, diagnose, and manage acute and chronic conditions, referring patients to specialist services as necessary.
  • Administer medications, perform minor procedures, and provide wound care.
  • Participate in clinical audit and quality improvement initiatives to ensure the delivery of safe and effective care.
  • Develop and maintain professional relationships with patients, carers, and healthcare professionals to promote seamless care transitions.
Requirements
  • Registered Nurse with current NMC registration.
  • Experience in primary care or community nursing, with a strong understanding of healthcare governance systems and risk management.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
  • Ability to prioritize tasks, manage time, and maintain accurate records.
  • Commitment to ongoing professional development and continuing education.
