Job Overview

Harrogate and District NHS Foundation Trust is seeking a skilled and compassionate individual to join our Home Care Team as a Home Care Worker. As a key member of our team, you will play a vital role in providing high-quality care and support to our service users in their own homes.

Key Responsibilities:

  • Provide personal care and support to service users, including assistance with daily living tasks, medication management, and health-related activities.
  • Work collaboratively with healthcare professionals and other stakeholders to develop and implement personalized care plans that meet the unique needs of each service user.
  • Conduct risk assessments and contribute to the development of care plans to ensure the safety and well-being of service users.
  • Administer medication in accordance with trust policies and procedures.
  • Support service users to maintain their independence and promote their overall health and well-being.
  • Communicate effectively with service users, their families, and other healthcare professionals to ensure seamless care coordination.
  • Participate in ongoing training and professional development to enhance your skills and knowledge in providing high-quality care.

Requirements:

  • Relevant NVQ level 3 or equivalent qualification.
  • 2 years of experience working in a health or social care environment.
  • Excellent communication and interpersonal skills.
  • Clerical and IT skills.
  • Literacy and numeracy skills to understand, deliver, and write care plans.
