An exceptional opportunity exists for a Band 5 Registered Nurse to become a vital member of our IV therapy community nursing team. Key Responsibilities

The primary duties of this role include:

  • Delivering care within the community, overseeing patients receiving intravenous therapy in their homes.
  • Conducting risk evaluations to determine the safety of patients remaining at home while undergoing treatment, and escalating concerns through established channels when necessary.
  • Implementing evidence-based clinical interventions to enhance health outcomes and promote patient autonomy.
  • Providing support to inpatient units and step-up care facilities as needed.
  • Acting as an advocate for patients, fostering choice, empowerment, and independence.
  • Collaborating with healthcare professionals and various agencies to ensure a cohesive and integrated service for our clients.

Clinical Responsibilities

  • Assessing, planning, implementing, and evaluating comprehensive patient care to facilitate safe IV medication administration at home.
  • Creating individualized, evidence-based care plans following holistic assessments, adjusting as needed.
  • Communicating complex and sensitive information regarding assessments, diagnoses, prognoses, and treatment plans to patients and caregivers.
  • Adapting explanations to overcome barriers to understanding, ensuring informed consent and cooperation.
  • Utilizing tact and persuasive skills to encourage patient compliance with care management plans.
  • Liaising with clinical leads and other healthcare professionals when clinical concerns arise.
  • Coordinating with various healthcare and social service providers to meet identified needs.
  • Utilizing assistive technology where appropriate.
  • Participating in multidisciplinary meetings as necessary.
  • Managing a caseload effectively, prioritizing according to service targets and patient needs.
  • Maintaining accurate patient records and ensuring timely data entry.
  • Demonstrating proficiency in using specialized equipment and performing advanced nursing skills.
  • Ensuring informed consent is obtained prior to initiating any interventions.
  • Adhering to infection control policies while managing exposure to bodily fluids.
  • Developing competencies to provide urgent responses to prevent unnecessary hospital admissions.

Qualifications and Experience

Essential:

  • Diploma or first-level degree in Nursing or equivalent.
  • Registered Nurse qualification.
  • Current registration with the Nursing and Midwifery Council (NMC).

Experience:

  • Demonstrated experience in community or acute care settings.
  • Knowledge of specialized areas through training and courses.
  • Experience in cannulation and IV therapy administration.
