Job Overview

We are seeking an experienced and ambitious Cardiac Rehabilitation Nurse to join our dynamic team at East London NHS Foundation Trust. As part of our Urgent Community Response and Virtual Ward initiative, you will play a crucial role in providing and leading efficient, integrated, urgent care to our patients with care and respect.

Main Duties of the Job

You will be working as part of the Primary Care at Home team, predominantly on Unplanned Care, to triage and assess referrals into the service. Your responsibilities will include initial history taking, clinical assessment, triage, prescribing (NMP qualification is essential for this role), and onward referral. You will also be accountable for working closely with other members of the MDT and partners to formulate a management plan that will support the patient to remain in the community.

Person Specification
  • Registered General Nurse
  • Educated to degree level or equivalent
  • Post-registration qualification in a relevant clinical field
  • Nurse Prescriber
  • Relevant teaching/mentoring qualification and evidence of teaching skills
  • Demonstrable evidence of significant post-registration nursing experience, including in a community setting
Key Skills and Qualities
  • Evidence of and commitment to multi-professional working
  • Demonstrate working within a multi-agency environment
  • Comprehensive understanding of partnership working
  • Application of clinical governance
  • Excellent understanding of community health and social care principles and practice
