Job Summary

We are seeking an experienced Field Care Supervisor to join our team, responsible for supporting the delivery of exceptional care services in the community. As a Field Care Supervisor, you will play a vital role in ensuring the seamless delivery of holistic packages of individual care support.

Key Responsibilities

  • Support the planning and delivery of care services in the community, working closely with care coordinators and other stakeholders.
  • Conduct regular reviews of individual services, including risk assessments and needs assessments.
  • Provide supervision, instruction, and support to care workers in the field, ensuring they have the necessary skills and knowledge to deliver high-quality care.
  • Participate in multi-disciplinary reviews of service users' needs, working collaboratively with healthcare professionals and other stakeholders.
  • Shadow new care workers in the field to ensure their competence and provide ongoing support and guidance.
  • Carry out on-site observations and evaluations of care worker practice, providing feedback and coaching to improve performance.

Requirements

  • Experience in carrying out initial risk and needs assessments in respect of individual service users.
  • Ability to work in partnership with service users, their family, carers, advocates, healthcare professionals, and others to ensure the seamless delivery of care services.
  • Strong communication and interpersonal skills, with the ability to build effective relationships with stakeholders.
  • Ability to work independently and as part of a team, with a flexible approach to working hours and locations.
