Job Summary
We are seeking an experienced Field Care Supervisor to join our team, responsible for supporting the delivery of care services in the community. As a Field Care Supervisor, you will play a key role in ensuring the seamless delivery of holistic packages of individual care support, working in partnership with service users, their families, carers, advocates, and healthcare professionals.
Key Responsibilities
- Support the planning and delivery of care services in the community, including the assessment and review of individual services and the practical supervision of care workers.
- Work with Care Coordinators to develop and implement effective care worker rotas, taking into account the needs of service users and the skills and attributes of care workers.
- Carry out regular reviews of individual services, including repeated and renewed needs and risk assessments, and participate in multi-disciplinary reviews of service users' needs.
- Shadow new care workers in the field to ensure their competence and provide on-site observation and evaluation of care worker practice as part of routine supervision.
Requirements
To be successful in this role, you will need experience in carrying out initial risk and needs assessments, working in partnership with service users, their families, carers, advocates, and healthcare professionals to ensure the seamless delivery of care services.
