Key Responsibilities

As a Health Advisor, your primary goal will be to ensure that Patients and Carers achieve optimal outcomes by delivering appropriate care at the right time and place.

You will be responsible for conducting Making Every Contact Count (MECC) assessments for patients and carers. Additionally, you will manage and organize all incoming MECC referrals, ensuring proper bookings into relevant services as necessary.

Your role will involve providing a high-quality, patient-centered experience within the Care Coordination Hub, supporting various functions to ensure exceptional service delivery to patients and carers. You will also offer focused telephone and administrative support to guide patients and carers in accessing the right services or healthcare professionals.

Person Specification

Experience

Essential

  • Prior experience in an office setting, executing general administrative tasks.

Desirable

  • Familiarity with patient appointment booking systems, particularly SystmOne.

Qualifications

Essential

  • NVQ Level 3 in Business Administration/Customer Services or equivalent experience.
  • Proficiency in word processing to RSA 3 standard or ability to demonstrate equivalent skills.

Knowledge

Essential

  • Comprehensive understanding of various administrative procedures and practices.
  • Proficient in a range of software applications, including Microsoft Word, Excel, Outlook, and EPR systems.

Desirable

  • Knowledge of SystmOne functionalities and other clinical reporting systems.
  • Understanding of clinic and appointment processes.
  • Familiarity with Making Every Contact Count (MECC) principles.
