We are seeking a highly motivated and organized individual to join our team as a Cancer Care Coordinator at James Alexander Family Practice. This is a full-time position, working across two sites, Bransholme Health Centre and Princes Medical Centre.
Key Responsibilities- Coordinate and integrate care for patients with cancer, ensuring seamless transitions between primary, secondary, and community care services.
- Act as a central point of contact for patients and practice staff, providing support and guidance on cancer care and treatment.
- Develop and implement systems to improve cancer screening uptake, working with practices to track and follow up non-attenders.
- Engage with hard-to-reach populations, forming strong working relationships with local community groups and voluntary sector organizations.
- Work alongside the Strategic Manager, Clinical Director, and Lead Care Coordinator to develop a non-clinical approach to cancer care reviews, ensuring proactive monitoring and tracking of patients suspected or confirmed of having a cancer diagnosis.
- GCSE grade A-C (or equivalent) in Maths and English or higher level qualification.
- 2-day personalized care institute accredited care co-ordinator training or be willing to undertake as part of the role.
- Experience of working in health, social care, and other support roles in direct contact with people, families, or carers.
- Excellent organizational skills, including planning, prioritizing, time management, and record keeping.
- Excellent communication skills, both written and verbal.
