Key Responsibilities
The primary function of the Health Advisor is to guarantee that Patients and Carers achieve optimal outcomes by delivering appropriate care at the right time and place.
You will facilitate a Making Every Contact Count (MECC) assessment for each patient or carer. Additionally, you will manage and organize all incoming MECC referrals and ensure proper bookings into relevant services when necessary.
Your role will be to provide a high-quality, patient-centered experience within the Care Coordination Hub, assisting all other departments in ensuring an exceptional patient and carer experience. This includes offering patient-focused telephone and administrative services that guide patients and carers in accessing the right services or healthcare professionals.
