We are seeking a highly skilled and experienced Occupational Therapist to join our community-based therapy team in South Molton. As a Specialist Community Occupational Therapist, you will play a key role in delivering high-quality patient-centered care to adults registered under South Molton and Wallingbrook medical centers.
Key Responsibilities- Provide specialist assessment, diagnosis, treatment, and advice to patients and their carers, managing a clinical caseload of patients with varying levels of complexity and health conditions.
- Work collaboratively with a multidisciplinary team, including occupational therapists, physiotherapists, occupational therapy apprentices, assistant practitioners, and therapy support workers, to ensure integrated and effective care.
- Develop and implement individualized care plans, taking into account the unique needs and goals of each patient.
- Contribute to the development and implementation of service-wide initiatives, ensuring that therapy input is integrated into patients' overall care plans.
- Provide supervision, training, and support to junior staff and students, promoting professional development and growth within the team.
- Participate in quality improvement programs, research, and audit activities, contributing to the advancement of occupational therapy practice within the Trust.
- Experience as an Occupational Therapist in a community-based setting, with a strong understanding of the needs and challenges faced by patients in this context.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and healthcare professionals.
- Strong clinical skills, with the ability to assess, diagnose, and treat patients with a range of health conditions.
- Ability to work autonomously, with minimal supervision, and to prioritize tasks effectively in a fast-paced environment.
