Qualified Youth Support Specialist - Transition Services

As a Youth Support Specialist in the Transition Services team, you will be instrumental in assisting young individuals in long-term care, whether through formal care arrangements or voluntary agreements.

Your key responsibilities will include:

  • Advancing long-term care strategies
  • Facilitating transition planning
  • Supporting youth as they navigate their journey to adulthood

Collaborating closely with families, foster caregivers, residential facilities, and semi-independent living arrangements, you will provide essential support to address the varied needs of the youth.


Utilize your expertise in statutory regulations, policies, and best practices to ensure the highest standards of care and support.

Demonstrate proficient IT capabilities to effectively manage caseloads and documentation.

Employ exceptional interpersonal skills to build and maintain constructive relationships with colleagues, partners, agencies, and, most importantly, the young individuals and their families.

Conduct assessments to determine the needs of youth and develop suitable intervention strategies.

Work collaboratively with multi-agency networks to ensure comprehensive care and support for the youth in your charge.

Handle legal matters under relevant legislation with professionalism and care.

Strive to achieve stability for youth, securing nurturing environments conducive to their development.

Actively contribute to the ongoing enhancement and progress of our Transition Services team.


Qualifications:

BA in Social Work, MA in Social Work, or equivalent qualifications.

Current registration with the relevant professional body.

You should possess comprehensive knowledge of statutory regulations, policies, and best practices related to youth in care.
