Job Summary

We are seeking a skilled and compassionate Community Nurse to join our team at North Uist Medical Practice. As a key member of our community nursing team, you will provide high-quality nursing care to patients in their own homes, working closely with other healthcare professionals to deliver seamless care.

Key Responsibilities

  • Provide comprehensive nursing care to patients in their own homes, assessing their needs and developing personalized care plans.
  • Work collaboratively with other healthcare professionals, including doctors, therapists, and social workers, to ensure coordinated care.
  • Conduct regular health checks and provide health education to patients, promoting healthy lifestyles and disease prevention.
  • Participate in on-call duties and work a flexible schedule, including weekends and evenings, to meet the needs of our patients.
  • Develop and maintain strong relationships with patients, families, and carers, ensuring their needs are met and their voices are heard.

Requirements

  • Registered Nurse with a valid UK driving license and access to a car.
  • Experience working in a community nursing setting, with a strong understanding of community health needs.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse groups.
  • Ability to work independently and as part of a team, with a flexible approach to work.
