Key Responsibilities- Provide administrative support to the multi-disciplinary team, ensuring the smooth operation of patient care services.
- Manage and maintain accurate and comprehensive data, including spreadsheets and reports, to support the Integrated Discharge Team's work.
- Collaborate with colleagues to ensure effective communication and coordination of patient care.
- Develop and maintain databases, spreadsheets, and other data management tools to support the team's work.
- Attend daily triage meetings and update the Sit Rep, ensuring timely and accurate information sharing.
- Monitor and track patient referrals, ensuring efficient and effective management of the discharge process.
- Good standard of numeracy and literacy, with GCSE level education or equivalent.
- Experience in administrative roles, with a strong background in data management and IT skills.
- Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
- Ability to prioritize workload and meet deadlines, with a strong attention to detail and organizational skills.
