Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our Medway Community Anticoagulation Service team. As a key member of our team, you will play a vital role in providing high-quality patient care and contributing to the delivery of excellent clinical outcomes.

Main Responsibilities
  • Accept written patient referrals and ensure accurate documentation on our anticoagulation service database.
  • Participate in computerized dosing of patients and collaborate with nursing colleagues, line managers, and medical staff as needed.
  • Address clinical problems and escalate concerns to medical staff or nursing line managers when necessary.
  • Provide a telephone and email advisory service to patients regarding their anticoagulation treatment.
  • Participate in new patient education clinics, delivering one-to-one patient education sessions for patients starting warfarin.
  • Train and become competent in using CoaguChek point-of-care test devices, participating in finger prick clinics, and training patients on INR monitoring.
  • Participate in the NEQAS external quality control scheme for near-patient testing analyzers, ensuring quarterly external quality controls are performed and reported.
  • Perform phlebotomy as required.
Key Requirements
  • Registered Nurse with community experience.
  • Educated to degree level or equivalent.
  • Anticoagulation qualification or willingness to undertake.
