About the Role

We are seeking two enthusiastic and highly motivated Senior Mental Health Practitioners to join our Home Treatment Team (HTT) at Berkshire Healthcare NHS Foundation Trust.

The successful candidates will have a passion for working with older adults and a strong understanding of mental health care. They will be responsible for assessing patients' needs, providing high-quality interventions, and working collaboratively with the multidisciplinary team.

Key Responsibilities
  • Assess patients' needs and develop individualized care plans
  • Provide high-quality mental health interventions, including care management
  • Work collaboratively with the multidisciplinary team to achieve positive outcomes
  • Contribute to the development and implementation of innovative solutions to improve patient care
Requirements
  • Relevant professional qualification (Social Worker, Mental Health Nurse, or Occupational Therapist)
  • Experience in caring for older adults with mental health needs
  • Strong understanding of mental health care and relevant legislation
  • Ability to assess risk and provide effective interventions
  • Valid UK driving license and use of car
