Berkshire Healthcare Foundation Trust is seeking a dedicated and experienced Mental Health Practitioner to join our dynamic team.
In this role, you will play a vital part in providing comprehensive mental health support to individuals within our community. Your expertise will be instrumental in assessing needs, developing personalized care plans, and delivering effective interventions.
Key Responsibilities:
- Conduct thorough assessments of mental health needs, formulating comprehensive care plans tailored to individual requirements.
- Collaborate effectively with multidisciplinary teams and external agencies to ensure holistic patient care.
- Provide direct support and guidance to service users, empowering them to manage their mental well-being.
- Demonstrate a strong understanding of relevant legislation and national guidelines within the field of mental health.
- Maintain accurate records and documentation, adhering to professional standards and ethical principles.
Essential Qualifications and Experience:
- Possess a recognized qualification in a relevant mental health discipline.
- Demonstrate extensive experience in conducting comprehensive mental health assessments and developing care plans.
- Have a proven track record of working effectively with individuals presenting high-risk presentations, including risk assessment and management.
- Exhibit exceptional communication skills, both written and verbal, enabling clear and concise interaction with clients, families, and colleagues.
Desirable Qualifications and Experience:
- Experience working with diverse community groups, including those facing complex needs, dual diagnoses, or safeguarding concerns.
- Familiarity with mental health settings supporting children, adolescents, and their families.
- Contribution to service improvement initiatives within a mental health context.
