About the Role

We are seeking a highly motivated and experienced Senior Mental Health Practitioner to join our Older Adults Mental Health Team at Berkshire Healthcare Foundation Trust.

Key Responsibilities
  • Assess and manage the mental health needs of older adults in a community setting
  • Develop and implement individualized care plans to promote recovery and well-being
  • Work collaboratively with multidisciplinary teams to provide comprehensive care
  • Conduct risk assessments and develop strategies to mitigate risks
  • Provide education and support to patients and their families
Requirements
  • Relevant professional qualification (Social Worker, Mental Health Nurse, Occupational Therapist)
  • Experience working with older adults in a mental health setting
  • Strong assessment and clinical reasoning skills
  • Ability to work autonomously and as part of a team
  • Valid UK driving license
