About the Role

We are seeking a highly motivated and caring Mental Health Practitioner to join our Community Older Adults Mental Health Team. As a key member of our team, you will be responsible for assessing and implementing intensive care and treatment for older adults with functional mental health problems and dementia.

This is a developmental role, offering the opportunity to progress to a B6 Mental Health Practitioner following a period of 18 months in supported practice and successful demonstration of competence.

You will work closely with our multidisciplinary team to provide specialist consultation and advice on issues relating to the care of older people with mental health problems to other professionals within the wider care community.

Main Responsibilities
  • Manage your own caseload, monitoring the mental health needs of patients and working alongside their carers to provide education and support.
  • Monitor and evaluate agreed care plans, making adjustments as appropriate in collaboration with the wider team.
  • Provide specialist consultation and advice on issues relating to the care of older people with mental health problems.
Requirements
  • Registered Mental Health Practitioner (Mental Health Nurse, Occupational Therapist, or Social Worker).
  • Experience of working with older people with mental health needs.
  • Dementia training.
  • Primary nursing experience.
  • Experience of working in the community.
