About the Role

We are seeking a highly motivated and enthusiastic Senior Mental Health Practitioner to join our Home Treatment Team (HTT) in Wokingham Community Hospital. As a key member of our team, you will be responsible for assessing the needs of older adults with mental health needs and providing high-quality interventions to support their recovery.

The successful candidate will have a passion for working with older adults and a strong understanding of the relevant legislation surrounding mental capacity and mental health. You will be able to assess risk in both routine and urgent situations and have a thorough knowledge of the network of support services for patients and carers.

Key Responsibilities
  • Assess the needs of older adults with mental health needs and provide high-quality interventions to support their recovery.
  • Work collaboratively with the multidisciplinary team to provide comprehensive care to patients.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Provide education and training to patients and carers on mental health and well-being.
  • Contribute to the development and implementation of policies and procedures to improve patient care and outcomes.
Requirements
  • Relevant professional qualification (Social Worker, Mental Health Nurse, Occupational Therapist) and clinical mental health experience.
  • Passion and understanding for working with older adults.
  • Good understanding of the relevant legislation surrounding mental capacity and mental health.
  • Ability to assess risk in both routine and urgent situations.
  • Thorough knowledge of the network of support services for patients and carers.
