About the Role

We are seeking a highly motivated and enthusiastic Mental Health Practitioner to join our Community Older Adults Mental Health Team in Wokingham. As a key member of our team, you will be responsible for assessing and implementing intensive care and treatment for older adults with functional mental health problems and dementia.

The successful candidate will have a passion for working with older people and a strong understanding of the Care Act, Mental Capacity Act, and Mental Health Act. You will be responsible for managing your own caseload, monitoring mental health needs, and working with carers and other professionals to assess care needs.

Main Responsibilities
  • Assess and implement intensive care and treatment for older adults with functional mental health problems and dementia.
  • Manage your own caseload and monitor mental health needs.
  • Work with carers and other professionals to assess care needs.
  • Provide specialist consultation and advice on issues relating to the care of older people with mental health problems.
Requirements
  • Registered Mental Health Practitioner (Nurse, OT, Social Worker, or Speech and Language Therapist).
  • Experience of working with older people with mental health needs.
  • Primary nursing experience.
  • Experience of working in the community.
  • Ability to plan, implement, and evaluate care plans.
  • Flexible and willing to undertake training in a specialist area as required.
