Mental Health Specialist

We are seeking a highly motivated and caring Mental Health Practitioner to join our Community Team at Berkshire Healthcare NHS Foundation Trust.
Job SummaryThe successful candidate will be responsible for managing their own caseload, monitoring the mental health needs of patients, and working alongside their carers and assessing care needs under the Care Act. Knowledge of appropriate legislation such as the Care Act, Mental Capacity Act, and Mental Health Act is essential for this role.
Main Responsibilities- Manage a caseload of patients with mental health needs, providing assessment, care planning, and intervention as required.
- Work collaboratively with other healthcare professionals, including social workers, nurses, and therapists, to provide comprehensive care to patients.
- Develop and implement care plans that meet the individual needs of patients, taking into account their physical, emotional, and social needs.
- Provide specialist consultation and advice on issues relating to the care of older people with mental health problems to other professionals within the wider care community.
- Monitor and evaluate the agreed care plan, making adjustments as appropriate in collaboration with the wider team.

Requirements- Registered Mental Health Practitioner - Nurse, OT, Social Worker, or Speech and Language Therapist.
- Valid UK driving license and use of a car for work purposes.
- Ability to work as a member of a team, with good communication and interpersonal skills.
- Flexibility and ability to work on own initiative, with a calm and professional demeanor in stressful situations.
