We are seeking a highly skilled and experienced Senior Mental Health Practitioner to join our team at Berkshire Healthcare NHS Foundation Trust. As a Senior Mental Health Practitioner, you will be responsible for providing high-quality care and support to patients with complex mental health needs.
Key Responsibilities- Formulate, execute, and evaluate care plans for patients with significant complexity within a multidisciplinary team.
- Identify specific interventions and skills required from the multidisciplinary team to deliver evidence-based care plans and expected health/social outcomes.
- Devise risk and safety plans with each individual patient and alert crisis/home treatment team services as required.
- Support patients to develop Wellness Recovery Action Plans and reduce dependence on secondary mental health services.
- Competent IT skills and ability to navigate various systems and software packages.
- Previous knowledge and experience of working in the community and with service users who have complex mental health needs.
- Experience of formulating risk assessments and working with mental health services.
- Ability to empathise, be supportive, and sensitive to the needs of others.
