About the Role

We are seeking a highly skilled and experienced Specialist Mental Health Practitioner to join our team in Primary Care. As a key member of our multidisciplinary team, you will be responsible for providing high-quality mental health services to adults and older adults with severe or significant mental illness.

Key Responsibilities
  • Assess and manage the mental health needs of adults and older adults in primary care settings.
  • Develop and implement personalized care plans to promote recovery and well-being.
  • Work collaboratively with primary care teams, community services, and voluntary sector organizations to deliver holistic health and social care outcomes.
  • Provide education and training to primary care staff on mental health issues and interventions.
  • Contribute to the development and implementation of mental health policies and guidelines in primary care settings.
Requirements
  • Current professional registration as a Registered Mental Health Nurse, Social Worker, Occupational Therapist, or equivalent.
  • Experience of working in community mental health services for adults and older adults with severe or significant mental illness.
  • Advanced understanding of mental health needs, person-centered care, and recovery-focused approaches.
  • Ability to work effectively in a team, manage own workload, and prioritize tasks.
  • Excellent communication and interpersonal skills.
