About the Role

We are seeking a highly skilled and experienced Specialist Mental Health Practitioner to join our team in Primary Care. As a key member of our multidisciplinary team, you will be responsible for providing high-quality mental health services to adults and older adults with severe or significant mental illness.

Key Responsibilities
  • Assess and manage clinical risk in individuals with mental health needs
  • Provide a range of evidence-based interventions to individuals with mental health problems and severe mental illness
  • Work collaboratively with primary care and community/voluntary sector partners to deliver holistic health and social care outcomes
  • Develop and implement care plans that promote recovery and independence
  • Contribute to the development and implementation of new services and initiatives
Requirements
  • Current professional registration as a Registered Mental Health Nurse, Social Worker, Occupational Therapist, or equivalent
  • Experience of working in community mental health services for adults and older adults with severe or significant mental illness
  • Advanced understanding of mental health needs and recovery-focused approaches
  • Ability to work effectively in a team and manage own workload
  • Excellent communication and interpersonal skills
