About the RoleThis is an exciting opportunity to be part of a new and innovative approach to mental health provision in Primary Care. You will work in a dynamic and people-centred environment, where you will have the opportunity to shape and improve provision for patients with complex mental health needs.
Key Responsibilities- Provide high-quality mental health care to patients in Primary Care settings
- Work collaboratively with multidisciplinary teams to develop and implement care plans
- Conduct assessments, interventions, and evaluations to support patient recovery
- Develop and maintain effective relationships with patients, families, and healthcare professionals
- Participate in service development and improvement initiatives
- Current professional registration: Registered Mental Health Nurse, Social Worker, Occupational Therapist, or equivalent
- Experience of working in mental health services for adults and older adults with severe or significant mental illness
- Ability to work effectively in a team and independently
- Strong communication and interpersonal skills
- Ability to work flexibly and adapt to changing circumstances
