Job Summary:

CWP is seeking a skilled Mental Health Practitioner to join our Home Treatment Team in Congleton. As a key member of our Urgent Care Mental Health First Response Services, you will work across various teams, including Crisis Resolution Home Treatment Teams, Liaison Services, Street Triage Services, Liaison and Diversion Services, and the Crisis Triage and Response Service.

Key Responsibilities:

  • Provide high-quality mental health care to individuals in urgent need, working collaboratively with other teams and services.
  • Develop and implement effective treatment plans, taking into account the unique needs and circumstances of each individual.
  • Contribute to the development and implementation of service improvements, ensuring that our services are responsive to the needs of our community.
  • Work closely with colleagues to ensure seamless transitions and continuity of care for individuals.
  • Participate in regular team meetings and contribute to the development of service policies and procedures.

Requirements:

  • Recognized qualification in mental health, such as RMN, AHP, Psychology, or Social Care (AMHPs).
  • Evidence of further professional development involving serious mental illness.
  • Non-medical Prescribing certificate.
  • Sound knowledge of current professional staff development and continuous service improvement issues, particularly in the context of ADHD and personality disorder services.
