Position Overview

An exciting opportunity has arisen for a Band 4 Rehabilitation Support Specialist to join a multidisciplinary community team, assisting therapists in providing exceptional rehabilitation services to patients.

This role is crucial for the ongoing care and management of clients within the community setting.

As part of this position, you will have the chance to develop your skills within community rehabilitation services, collaborating with a broader support network and contributing to an integrated care system. You will engage with our Discharge to Assess beds and across various care communities to address patients' rehabilitation and therapy requirements.

The primary location for this role will be within designated community areas, with flexibility to support colleagues across the wider care partnership as necessary.

The ideal candidate will possess an Assistant Practitioner qualification (ST0215 Healthcare Assistant Practitioner Level 5) or an equivalent credential. If you do not currently hold this qualification, there is an opportunity to pursue it while working at Band 3, transitioning to Band 4 upon successful completion.

Main Responsibilities

The Band 4 Rehabilitation Support Specialist will be accountable for managing a defined patient caseload within their Care Community, following assessment by a qualified therapist. This includes conducting independent assessments, formulating, and advancing individualized rehabilitation plans. You will work collaboratively with the team to deliver a comprehensive rehabilitation service under the supervision of qualified professionals for patients in their community therapy services.

Comprehensive induction upon starting the role, supervision from senior physiotherapists and occupational therapists, and personalized objective setting through appraisals and competencies. All therapy assistants benefit from regular supervision sessions, which will commence bi-weekly. Job Responsibilities

Service Delivery

  • Establish therapeutic connections with newly referred patients as identified by qualified therapists following admission and assessment.
  • Independently conduct specified holistic assessments, interventions, and develop rehabilitation programs either autonomously or in collaboration with a qualified clinician for community-based patients.
  • Adhere to departmental protocols and guidelines, consulting with the qualified clinician as needed.
  • Manage a caseload as assigned by the qualified clinician, providing updates at agreed intervals and highlighting necessary actions. Work independently while supporting Band 3 Rehabilitation Support Specialists as required.
  • Engage with individuals in various environments, ensuring patient safety and dignity are consistently upheld.
  • Adapt interventions and programs within defined parameters and scope of practice.
  • Conduct initial assessments and procure basic equipment and adaptations as appropriate for service users, completing necessary risk assessments prior to prescribing equipment and reviewing them regularly.
  • Oversee the ordering and inventory management of materials and equipment as needed for the unit.
  • Instruct patients, caregivers, and other professionals on the safe use and maintenance of equipment, including simple aids such as walking frames and functional aids.
  • Refer patients to appropriate services and healthcare professionals when complex health needs are identified.
  • Facilitate patient discharges within your caseload after consultation with a qualified team member.
  • Participate in multidisciplinary meetings and contribute to the clinical decision-making process for individual patients as delegated by qualified staff.
  • Maintain accurate and up-to-date clinical records, ensuring their security and confidentiality.
  • Operate within your competency and seek guidance from qualified therapists when necessary.
  • Participate (pro-rata) in service provision from 8 am to 8 pm, seven days a week, to support the Urgent Care Response service.
