Position Overview
An exciting opportunity has arisen for a Band 4 Rehabilitation Support Specialist to join a multidisciplinary community team and assist therapists in providing exceptional therapeutic interventions to patients.
This role is crucial for the ongoing care and management of clients within the community setting.
The position offers a chance to develop skills within community therapy services, connecting with a broader support network and being part of an integrated care system. You will collaborate across various care communities to address patients' rehabilitation and therapy requirements.
While the primary location for this role will be within designated areas, there may be a need to support colleagues across the wider service footprint as required.
The ideal candidate will possess an Assistant Practitioner qualification (ST0215 Healthcare Assistant Practitioner Level 5) or an equivalent qualification. If you do not currently hold this qualification, there is an opportunity to work towards it, initially at band 3, with a progression to band 4 upon successful completion.
Main Responsibilities
The Band 4 Rehabilitation Support Specialist will manage a defined caseload of patients within their Care Community, following assessment by a qualified therapist. This includes conducting independent assessments, formulating, and advancing a personalized rehabilitation plan. You will work collaboratively within the team to deliver a comprehensive rehabilitation service under the supervision of qualified staff for patients in the community therapy services.

We provide a comprehensive induction upon starting the role, along with supervision from senior physiotherapists and occupational therapists. Individual objectives will be set through appraisals, and all therapy assistants benefit from regular supervision sessions. Key Responsibilities
Service Delivery
- Establish therapeutic contact with newly referred patients as identified by qualified therapists.
- Conduct specified holistic assessments and interventions, creating rehabilitation programs independently or in collaboration with qualified clinicians for community-based patients.
- Adhere to departmental protocols and guidelines, seeking advice from qualified clinicians as necessary.
- Manage a caseload as delegated by qualified clinicians, providing updates and highlighting necessary actions.
- Work with individuals in various settings, ensuring patient safety and dignity are maintained.
- Adjust interventions and programs within defined parameters and scope of practice.
- Complete initial assessments and procure necessary basic equipment and adaptations for service users.
- Manage ordering and stock control of materials and equipment as required.
- Instruct patients, caregivers, and other professionals on the safe use and maintenance of equipment.
- Refer patients to appropriate services and healthcare professionals for complex health needs.
- Discharge patients from your caseload after consultation with qualified team members.
- Participate in multidisciplinary meetings and contribute to clinical decision-making processes as directed by qualified staff.
- Maintain accurate and up-to-date clinical records, ensuring their security and confidentiality.
- Work within your competency and seek guidance from qualified therapists when necessary.
- Participate in service delivery across a flexible schedule to support urgent care response services.

Qualifications
Essential
- Assistant Practitioner qualification (ST0215 Healthcare Assistant Practitioner Level 5) or willingness to work towards completion.
