An exciting opportunity has arisen for a Senior Mental Health Practitioner to join our team at Pennine Care NHS FT in Tameside & Glossop. As a key member of our Living Well service, you will work collaboratively with individuals in a community setting to help them navigate and connect to a range of mental health services, community activities, and services in line with their individual needs, circumstances, and goals to enhance their health, wellbeing, and quality of life.
Main Duties of the JobAs a Senior Mental Health Practitioner, you will conduct biopsychosocial assessments and initial conversations with people presenting with mental health difficulties, social care needs, and risk issues at a primary care level. Your aim will be to promote recovery and help people maintain their lives within their community environment.
Key Responsibilities of the Post:You will assess risk, devise risk management plans with service users as part of the Duty function, promoting self-management to enable service users' engagement with other parts of LWT and offering advice to colleagues where necessary.
You will also be able to offer brief interventions (max 12 weeks) including devising coping strategies, problem-solving, motivational and behavioural activation work, anxiety management, risk management, and suicide prevention work, medication support, promoting social and interagency support, providing psychological support and biopsychosocial education aimed at insight development, promoting self-help through resources and signposting, and reviewing treatment plans with service users deemed too complex for other parts of LWT or who may not meet the criteria for secondary care services.
Person Specification:Applicants must possess RMN/RNMH/Social Worker/OT or equivalent. Completion of Preparation for Mentorship/Practice Teacher/Fieldwork Educator qualification or willingness to complete is desirable, together with significant post-registration experience.
Essential skills and abilities include comprehensive mental health and risk assessment skills, ability to deliver evidence-based interventions, excellent communication skills, both written and verbal, and the ability to form productive working relationships with service users, carers, and multi-disciplinary staff.
Desirable skills include skills in planning, delivery, and evaluation of training, skills in the area of audit or research, and experience of working with a psychological model of care.
