Job Description: Community Support Specialist

We are seeking a dedicated and compassionate Community Support Specialist to join our team. In this role, you will play a vital part in delivering person-centered support to individuals within their homes and communities.

Responsibilities:

  • Provide practical and emotional support to individuals, families, and carers navigating various health and social care challenges.
  • Conduct needs assessments and develop personalized support plans tailored to individual circumstances.
  • Facilitate access to community resources and services, connecting individuals with relevant organizations and support networks.
  • Offer guidance and encouragement, empowering individuals to achieve their goals and maintain independence.
  • Maintain accurate records and documentation, ensuring compliance with all relevant policies and procedures.

Qualifications and Experience:

  • Proven experience working in a community development setting, adult health & social care, learning support, or public health/health improvement.
  • Demonstrated ability to effectively communicate with individuals from diverse backgrounds, both remotely and face-to-face.
  • Strong organizational and time management skills, with the ability to prioritize tasks and meet deadlines.
  • A commitment to person-centered care and a passion for supporting individuals in achieving their full potential.
