About the Role

We are seeking a highly motivated and enthusiastic Mental Health Practitioner to join our Barrow Home Treatment Team. As a key member of our team, you will provide comprehensive assessments and triage services to individuals in acute mental health distress, supporting them to avoid hospital admission and facilitating early discharge from hospital.

Key Responsibilities
  • Provide evidence-based mental health assessments and signposting to alternative service provision for individuals requiring specialist urgent mental health assessment.
  • Collaborate with multidisciplinary teams, including GPs, Police, and Acute Hospital colleagues, to provide comprehensive, collaborative triage services.
  • Develop and implement crisis plans and contingency planning to support individuals in crisis.
  • Work sensitively and non-judgmentally with service users, carers, and multidisciplinary teams to provide high-quality care and support.
Requirements
  • Registered Mental Health Nurse, Social Worker, or Occupational Therapist.
  • Strong communication and negotiation skills, with the ability to work effectively with service users, carers, and multidisciplinary teams.
  • Motivational and assertiveness skills, with the ability to work sensitively in a non-judgmental way with service users in crisis.
