About the Role

We are seeking a highly motivated and compassionate individual to join our Mental Health Team as a Mental Health Practitioner. As a key member of our team, you will play a vital role in supporting the delivery of recovery care plans for clients with a range of mental health presentations in the community.

Key Responsibilities
  • Work as part of the multi-disciplinary team to support the delivery of recovery care plans for clients with mental health presentations in the community.
  • Facilitate discharges from hospital and support the safe administration of ECT.
  • Work intensively with people to support them in their home, preventing hospital admission and facilitating hospital discharge.
  • Support the assessment of carers as part of a Devon Memory Service assessment and ensure safe transfer and administration of ECT to patients while inpatients.
Person Specification
  • Basic Education
  • QCF Level 3 in Health and Social Care or equivalent qualification
  • Care Certificate training
  • Previous care experience with people who have Mental Health difficulties
  • Awareness of safeguarding principles
  • Enhanced DBS Check
