We are seeking a highly motivated and compassionate individual to join our Mental Health Team as a Mental Health Practitioner. Key Responsibilities- Work as part of the multi-disciplinary team to support the delivery of recovery care plans for clients with mental health presentations in the community.
- Facilitate discharges from hospital and support the safe administration of ECT.
- Work intensively with people to support them in their home, preventing hospital admission and facilitating hospital discharge.
- Support the assessment of carers as part of a Devon Memory Service assessment and ensure safe transfer and administration of ECT to patients while inpatients.
- Basic Education
- QCF Level 3 in Health and Social Care or equivalent qualification
- Care Certificate training
- Previous care experience with people who have Mental Health difficulties
- Awareness of safeguarding principles
- Enhanced DBS Check
