Job Summary

We are seeking a skilled Community Health Specialist to join our team at Torbay and South Devon NHS Foundation Trust. The successful candidate will work closely with our specialist team to provide high-quality care to housebound patients in Paignton and Brixham.

Main Responsibilities
  • Deliver high-quality care to patients in their own homes, working collaboratively with primary care and secondary care teams.
  • Conduct Specialist Nursing and Continuing Care assessments, utilizing relevant training and expertise.
  • Develop and implement holistic care plans, taking into account the unique needs and circumstances of each patient.
  • Work effectively in an integrated health and social care environment, promoting a healthy work/home life balance.
Requirements
  • Qualified Registered Nurse Level 1
  • Evidence of continuing professional development, including Community Nurse Prescriber V150 and relevant I.T. knowledge/experience.
  • Ability to communicate complex information to patients, carers, and health and social care staff, with a focus on patient-centered care.
  • Full driving license and access to transport, with a willingness to undertake V150 community practitioner prescribing course.
