Job Summary

We are seeking a skilled and dedicated Healthcare Practitioner to join our team at Torbay and South Devon NHS Foundation Trust. As a key member of our community Paignton Health and Well-being Team, you will play a vital role in delivering high-quality care to housebound patients.

Main Duties

You will work as part of a multi-disciplinary team, partnering with primary care and secondary care to provide comprehensive care to our patients. Your expertise will be invaluable in supporting the development of less experienced staff and contributing to service development.

Requirements

To be successful in this role, you will need to have a Foundation Degree (FdSc in Healthcare Practice (Nursing Associate)) and Trust induction training. You will also need to demonstrate evidence-based practice, understanding of when to seek advice and refer to a registered care professional, and intermediate IT skills.

Qualifications and Training

  • Foundation Degree (FdSc in Healthcare Practice (Nursing Associate))
  • Trust induction training

  • Experience in a Community Setting

Knowledge and Experience

  • Understand and demonstrate evidence-based practice
  • Understanding of when to seek advice and refer to a registered care professional
  • Understanding of the scope of the role of the Nursing Associate in context of the team and the organisation, and how the role may contribute to service development
  • Intermediate IT skills
  • Understand and demonstrate the importance of the promotion of health and wellbeing

Specific Skills

  • Ability to work independently with a delegated caseload, use initiative whilst understanding limits of scope
  • Ability to work effectively as a team player under appropriate supervision, and as part of a multidisciplinary team
  • Ability to support the development of less experienced staff
