Job Title: Clinical Health Assessor

The successful candidate will be responsible for assessing individuals in the community, home, nursing homes, and hospitals to determine eligibility for NHS Continuing Healthcare or Funding Nursing Care. As a member of the multidisciplinary team, the assessor will coordinate the process, establish eligibility, and assist in identifying a suitable care package.

Main Responsibilities:
  • Utilize national assessment tools and adhere to required timescales to coordinate and participate in the assessment process with the multidisciplinary team.
  • Determine individual eligibility in line with the NHS Framework for NHS Continuing Healthcare and Funded Nursing Care.
  • Explain the process to patients, families, health and social care colleagues, and care providers, ensuring transparency and clear understanding.
Key Skills and Qualifications:
  • Excellent verbal, written, and presentation skills.
  • Computer literacy and ability to use software programs to maximize contribution to the post.
  • Demonstrated knowledge of current legislative changes affecting the NHS and the Local Authority.
  • Application of the NMC Code of Conduct and Scope of Professional Practice.
  • Project management skills to produce outputs within set timescales.
Working Arrangements:

The post requires the ability to work weekends as per rota and maintain professional integrity. The successful candidate will be accessible and flexible to support the needs of the service.

This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such, a submission for Disclosure to the Disclosure and Barring Service (formerly known as CRB) will be required to check for any previous criminal convictions.
