Job Summary

We are seeking a dedicated and compassionate Female Support Worker to join our team at Uplands House Care Home in Fareham. As a key member of our caregiver team, you will provide high-quality care and support to residents with mental health and physical health needs.

Key Responsibilities
  • Provide care and support to residents with all aspects of daily living, following their individualized care plans and promoting their quality of life and well-being.
  • Support residents with their physical health needs, including personal care, and their mental health needs, promoting healthy life choices and social inclusion.
  • Assist the nursing team in developing residents' care plans to be person-centered and inclusive.
  • Work towards attaining residents' independence and choice in their daily lives, following their individual programs and operational policies.
  • Support residents to integrate and gain new experiences within their local community.
  • Establish and maintain relationships with residents, promoting a pleasant and therapeutic environment.
  • Accompany residents on shopping trips and outings.
  • Participate in the preparation and serving of meals with residents, taking into account their personal preferences and dietary needs.
  • Take part in scheduled day activities with residents, promoting social interaction and a sense of community.
  • Act as a co-key worker for a resident, supporting them in achieving their goals and promoting their well-being.
Requirements
  • Experience working in the care sector, specifically with individuals with mental health and physical health needs.
  • Familiarity with safeguarding policies and procedures.
  • Clear understanding of personal development plans and the importance of ongoing training and education.
  • Ability to work as part of a team, promoting a positive and supportive work environment.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with residents, colleagues, and other stakeholders.
