We are seeking a dedicated and compassionate Female Support Worker to join our team at Uplands House Care Home in Fareham. As a key member of our caregiver team, you will play a vital role in providing high-quality care and support to our residents with mental health and physical needs.
Key Responsibilities- Provide care and support to residents with all aspects of daily living, following their individualized support plans and promoting their quality of life and wellbeing.
- Support residents with their physical health needs and mental health needs, including personal care and promoting healthy life choices.
- Promote residents' physical wellbeing through regular health checks and social inclusion.
- Assist the nursing team in developing residents' support plans to be person-centered and inclusive.
- Work towards attaining residents' involvement, independence, and choice in their daily lives, following their individual programs and operational policies.
- Support residents to integrate and gain new experiences within their local community.
- Establish and maintain relationships with residents to complement and contribute to their recovery care program.
- Accompany residents on shopping trips and outings.
- Participate in the preparation and serving of meals with residents, taking into account their personal preferences and dietary needs.
- Take part in scheduled day activities with residents in accordance with their recovery program.
- Act as a co-key worker for a resident with support from the key worker/deputy/hospital manager.
- Demonstrate a willingness to be respectful, accepting, and tolerant of the people requiring support, ensuring their dignity is maintained at all times.
