About the Role

We are seeking a dedicated and compassionate Reablement Support Worker to join our team in North Norfolk. As a Reablement Support Worker, you will play a vital role in helping individuals regain their independence and confidence in their own homes.

Key Responsibilities
  • Provide person-centred support to individuals in their own homes, promoting independence and confidence.
  • Assist with daily activities such as washing, dressing, meal preparation, and medication management.
  • Work collaboratively with the Reablement Team to develop and implement personalized care plans.
  • Communicate effectively with individuals, families, and healthcare professionals to ensure seamless care.
  • Contribute to a positive and supportive team environment, promoting a culture of excellence and compassion.

Requirements
