About the Role: Support Worker

We are seeking dedicated and empathetic Support Workers to join our Theresa House and Hamlets Way services in Tower Hamlets. In this role, you will play a vital part in empowering individuals to live fulfilling lives by providing personalized care and support.

Your Responsibilities:

  • Develop and implement effective support plans tailored to individual needs.
  • Provide compassionate assistance with daily living activities, such as social engagement, physical well-being, medication management, and benefit entitlements.
  • Support customers in exploring educational, vocational, and personal growth opportunities.
  • Collaborate with team members to ensure a holistic and coordinated approach to care.
  • Maintain accurate records and documentation, adhering to organizational policies and procedures.
