About the Role

We are seeking a highly skilled and experienced professional to join our team as a Children's Continuing Care Deputy CNS at Guy's and St Thomas' NHS Foundation Trust. This is an exciting opportunity to work in a dynamic and supportive environment, providing high-quality care to children and young people with complex needs.

Key Responsibilities
  • Lead the Continuing Care Team in the coordination of care for children and young people with complex life-threatening or life-limiting conditions.
  • Develop and implement individualized care plans in collaboration with families, healthcare professionals, and other stakeholders.
  • Provide expert clinical guidance and support to colleagues, ensuring high-quality care and patient outcomes.
  • Collaborate with the multidisciplinary team to ensure seamless transitions of care and effective discharge planning.
  • Manage and maintain accurate records of patient care, ensuring confidentiality and adherence to trust policies.
  • Participate in education and training programs to enhance clinical knowledge and skills, promoting excellence in patient care.
  • Contribute to the development and implementation of policies, procedures, and guidelines, ensuring compliance with trust standards.
  • Act as a role model, demonstrating specialist knowledge, leadership, and high standards of clinical practice.
Requirements
  • Registered Nurse (Child) with a BSC/1st degree (health-related) or equivalent qualification/training and experience.
  • Post-registration qualification in the relevant specialty.
  • Evidence of professional/clinical knowledge in the area, supplemented by specialist clinical, managerial training, and CPD.
  • Mentorship Course or equivalent.
  • Leadership qualification (LEO) desirable.
Desirable Skills and Experience
  • Previous post-registration experience within the relevant specialty.
  • Experience of teaching, assessing, and supervising others in a clinical setting.
  • Experienced and competent in the management of patients within the specialty.
