About the Role

We are seeking a skilled and compassionate Continuing Care Assistant to join our Community Children's Nursing team in Greenwich. As a key member of our team, you will provide high-quality, specialist care to children and families with complex health needs in their own homes and community settings.

Key Responsibilities
  • Develop and implement individualized care plans for children with complex health needs, working closely with the Lead Continuing Care nurse and Assistant Practitioner.
  • Provide skilled clinical care, including tracheostomy care, enteral feeding, and basic life support, following competency-based training.
  • Ensure a safe and supportive environment for children and families, maintaining high standards of personal care and hygiene.
  • Work collaboratively with the multidisciplinary team to provide holistic care, including play and stimulation to meet emotional developmental needs.
  • Identify and report concerns to the Continuing Care nurse, implementing agreed individual programs of care and acting as a key worker if required.
Person Specification

We are looking for an individual with:

  • NVQ 3 or the Cavendish Care certificate.
  • Experience working with children with disabilities and complex health needs.
  • Ability to work as part of a team, with excellent communication and interpersonal skills.
