Physiotherapist - Primary Care Specialist

We are seeking a highly skilled and experienced Physiotherapist to join our team at Southend East Primary Care Network. As a Primary Care Physiotherapist, you will play a vital role in delivering high-quality patient care and contributing to the development of our musculoskeletal services.
Key Responsibilities- Clinical Practice
- Assess, diagnose, and manage patients with musculoskeletal conditions, taking responsibility for a complex caseload.
- Develop and implement treatment plans, including independent prescribing and injection therapy, when trained to do so.
- Manage complex interactions, including working with patients with psychosocial and mental health needs, referring to social prescribing when appropriate.
- Deliver programmes of supported patient self-management, facilitating behavioural change and optimising physical activity, mobility, and independence.
- Provide care tailored to individual needs, appraising the impact of clinical status on general health, well-being, and employment status.
- Evidence, Research, and Development
- Develop relationships and a collaborative working approach across the PCN, supporting the integration of pathways in primary care.
- Work as part of an MDT in a patient-facing role, using expert knowledge of MSK issues to create stronger links for wider MSK services.
- Support research and audit programmes to evaluate and improve the effectiveness of our FCP service.
- Streamline the pathway of care by providing a responsive service, ensuring timely access to care.
- Leadership
- Lead, manage, and contribute to service delivery, implementing effective clinical governance.
- Communicate complex and sensitive information to patients and carers, supporting personalised care.
- Facilitating Learning
- Provide feedback and support to other PCN clinical team members on referral criteria.
- Critically reflect on your own practice, continuing to develop your skills.
- Qualifications
- Fully HCPC and CSP registered practicing Physiotherapist.
- Experience
- Minimum 5 years post-graduate physiotherapy experience.
- Minimum 3 years of clinical experience delivering musculoskeletal assessment and treatment.
- ARRS Requirements
- Completed an undergraduate degree in physiotherapy.
- Registered with the Health and Care Professional Council.
- Holds relevant public liability insurance.
- Masters Level qualification or equivalent specialist knowledge, skills, and experience.
- Can demonstrate working at Level 7 capability in MSK related areas of practice or equivalent.
