Primary Care Careers is seeking a skilled Paramedic to join our Integrated Neighbourhood Team. This is a 9-month fixed-term contract, with the possibility of being full-time or part-time.
Main Responsibilities- Assess, diagnose, plan, implement, and evaluate treatment/interventions and care for patients presenting with undifferentiated diagnoses.
- Triage patients within the primary care network site.
- Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
- Conduct weekly visits to care homes to carry out ward rounds.
- Work through a home visit list for patients requested by member surgeries, attend to patients, and assess them based on the GP surgery's request.
- Utilize the FrEDA assessment to ensure patient details are easily accessible to other healthcare providers.
- Work closely with the Population Health Management team to ensure the primary care network integrates with other service providers.
- Carry out QOF reviews for surgeries for patients unable to attend.

Job Requirements- Degree/diploma level in Paramedicine or equivalent.
- Registered with the Health and Care Professions Council (HCPC).
- Completed the two-year Consolidation of Learning period as a newly qualified paramedic.
- Further three years' experience as a band 6 (or equivalent) paramedic.
- Previous experience working within a primary care environment.
- Triage experience in a primary care setting.
