About the Role

We are seeking a highly skilled and experienced pharmacist to join our team at Southend East Primary Care Network. As a Clinical Pharmacist - Medicines Optimisation Specialist, you will play a key role in expanding our pharmacy team and contributing to the delivery of high-quality patient care.

Key Responsibilities
  • Manage medicines after discharge from hospital, ensuring seamless transitions and optimal patient outcomes.
  • Risk stratification: Identify patients at high risk of harm from medicines and develop targeted interventions to mitigate these risks.
  • Unplanned hospital admissions: Analyze data to identify patients at risk of unplanned hospital admissions due to medication-related issues and implement strategies to prevent these admissions.
  • Repeat prescribing: Oversee the repeat prescribing reauthorisation process, ensuring timely and accurate reviews of patient requests.
  • Patient-facing medicines support: Provide expert advice and guidance to patients on medication management, adherence, and safety.
  • Care home medication reviews: Conduct regular medication reviews for patients residing in care homes, ensuring optimal medication use and minimizing risks.
  • Long-term condition clinics: Participate in clinics focused on managing patients with chronic conditions, providing expert medication management and guidance.
  • Service development: Collaborate with the team to develop and implement new services aligned with emerging medicines and NICE guidance.

Person Specification
  • Essential:
    • Mandatory registration with the General Pharmaceutical Council.
  • Desirable:
    • Membership of the Royal Pharmaceutical Society.
    • Masters degree in pharmacy (MPharm).
    • Clinical diploma.
    • Clinical assessment skills.
