About the Role

We are seeking an experienced and enthusiastic nurse to join our team as a Community Heart Function Nurse Specialist. This is a Band 6 position based at The Coley Clinic in Reading, where you will work as part of a wider integrated Cardiac and Respiratory Specialist Services (CARSS) covering the Berkshire West locality.

Key Responsibilities
  • Facilitate and oversee community-based Heart Failure clinics in various community settings, including patients' homes.
  • Work flexibly in response to service need across multiple sites in Berkshire West.
  • Take responsibility for case management of patients with heart failure in the community setting.
  • Support colleagues in the delivery of compassionate, safe, and structured care.
  • Make differential diagnosis within level of competence.
  • Plan and provide treatment in collaboration with others as appropriate.

Requirements

To be considered for this role, you must have:

  • Recent clinical experience within the specialty.
  • Demonstrate effective chronic disease management.
  • Ability to effectively manage challenging behavior and/or conflict.
  • Car driver with ability and readiness to travel to multiple sites.
