Key Responsibilities

As a member of our Community Nursing Team, you will assess, support, and care for patients with diverse health requirements, ensuring high-quality, evidence-based nursing practice. Your enthusiasm for delivering care in patients' homes will be crucial. Collaborating with team members, you will holistically evaluate the needs of patients and their families, working together to develop, implement, and review nursing care plans.

Your duties will include:

Conducting initial assessments and taking necessary actions, reporting to the Team Leader for further evaluation. Delivering care based on best practice evidence and collaborating with colleagues to enhance evidence-based practices. Actively promoting self-care among patients and integrating health promotion into care plans. Participating in organized educational programs for patients.

Essential Qualifications

Current NMC registration or nearing qualification. Valid UK driving license and access to a vehicle for work purposes. *Please note that not all locations may be reachable by public transport*.
