We are seeking a skilled and compassionate Community Staff Nurse to join our Out of Hours District Nursing team at Berkshire Healthcare NHS Foundation Trust.
Key Responsibilities- Assess the needs of patients and carers, and work in partnership with them to plan, implement, and evaluate programs of nursing care.
- Undertake first visits and take immediate action, reporting back to the Team Leader or Community Nursing Band 6/Community Matron for further discussion and/or assessment.
- Establish working relationships with other professionals and voluntary agencies to achieve health aims.
- Provide care according to the available evidence of best practice and support the development of evidence-based practice.
- Registered General Nurse (RGN) with NMC registration.
- Evidence of Continuous Professional Development (CPD).
- Ability to assess, plan, implement, and evaluate care.
- Well-developed written and verbal communication skills.
- Ability to work independently and as part of a team.
- Full UK driving license and access to own car.
